RHD - Sample OSPE STATION
Submitted by M.R.T. on Mon, 15/06/2009 - 1:27am.
Here is a 68 year old female who died of LVHF (left ventricular heart failure), reported in her file was recurrent pharyngitis / Tonsillitis during childhood (age 8-15).
She also had Hypertension and Diabetes before she passed away.
After having a look at pictures from her heart shown Below:
1) Which valve has been affected?
2) Which valve is most likely also affected in patients with similar history?
3) explain the sequence that lead to her death, assuming this was incompetence?
4) what is shown in the 1st picture ( Gross of her heart on autopsy)?
Give name and description of these masses?

5) what is show below in the picture taken form her myocardium?
Describe these nodules in terms of cellular content?
MY ANSWERS:
1) Mitral
2) Aortic
3) Mitral incompetence alone lead to dilatation in all 4 chambers of the heart, and the most functional one ( LV) will be the first to fail.
4)vegetations occurring in RHD are called MCcallum's Patches
Pathologically described as small multiple masses
5) Aschoff's Nodules, contain inflamatory cells, lymphocytes and Plasma cells, found in the myocarduim with occasional OWL eye

PAthogenesis of RHD:
